Update migrations.rst

I was reviewing the migrations doc for my personal project and thought I'd try to spend some time adding a few things and tweaking a few others. I'll try to work my way through the remainder of the document, but I wanted to send this in before I lost it. Let me know if you spot anything I should alter.
