Add some formatting, disambiguation and one note

Add note in Creating Form Inputs section to remind that input method generates wrapping divs.
Add small formatting in the same section - wrap in code tags all pure text element or method names to make page consistent with itself
Disambiguate and/or clarify small phrase parts in the same section.
