Add arguments and arguments descriptions for create()

Change "next" with "a following" in Starting a Form, as the section referring to Context doesn't come first after that paragraph
Add empty lines between $model built-in providers enumeration to improve readability
Add links to context provider classes in each enumeration
Add new sub-section in Starting a Form, named Options for Form, which contains the valid options for create() method
