Fix hyperlinks, section title, indentation

Fix broken reST hyperlinks in Starting a Form section and for the wikipedia link.
Change subsection title to Options for Form Creation.
Remove quotes on 'type' key name in Changing the HTTP Method for Form section.
Capitalize Modelname.fieldname across the whole page.
Fix indenting of code block examples to always follow 4 spaces multiples in respect to page left margin or in respect to the paragraph hanging indent (when inside enumeration lists).
Fix 2 other small formattings in the text.
